Know what to do when it’s time to leave hospital

Your health care team will involve you in planning your discharge from hospital.  You will be provided with a discharge folder, with important information for you to refer to. You will also find helpful information in theicon Leaving St Andrew's Hospital brochure.

When you are ready to leave hospital:

  • Take your discharge folder

  • Know who to contact next and where to go

  • Know about your medications

  • Know how to care for your wound (if applicable)

  • Know about any community services that have been organised
